Eligibility Documents List

Below is a list of the documents that will assist in determining your eligibility for our Head Start program. 

• Original Birth Certificate for child(ren).
• State ID with your current address.
• Enrolling child(ren)’s medical card. The Benefits letter cannot be accepted.
• Income documentation that includes: SNAP, TANF, SSI documentation of benefits or income.

Other Documents Needed Prior to Enrollment

• Health Action Plan completed by the child’s pediatrician or specialist for chronic health conditions such as asthma, eczema, heart conditions, seizures, etc.
• Current Physical (or Well Child Exam) for your child’s age with labs (lead, TB, hematocrit).
• IFSP/IEP or other evaluations or diagnosis documentation.
